Job Description
Position Title: Clinical - LTSS Service Care Manager
Location: Source from Edinburg, McAllen, Elsa
Schedule: 8:00 AM 5:00 PM
Duration: 6 Months
Pay rate: XX25-27.00XX
Licensure Required:
~ LVN License
Day-to-day responsibilities:
Assist in telehealth biannual assessments, in person as needed if in the area.
Complete assigned tasks and document findings.
Coordination with multi-disciplinary teams.
Follow up calls to members.
Any other projects that may arise.
Must be able to meet strict turn-around times with documentation for assessments assigned.
Must haves:
~ Microsoft Word, Excel experience, Computer literate
Nice to haves:
~ At lease 2 years experience at LVN, Home Health experience, Case management experience
